Preoperative echocardiography for patients with hip fractures undergoing surgery: A retrospective cohort study using a nationwide database
Anesthesia & Analgesia Jan 23, 2019
Yonekura H, et al. - The link between preoperative echocardiography and postoperative morbidity as well as patient survival was investigated in 66,620 patients who underwent hip fracture surgeries, with preoperative echocardiography screening performed in 34,679 (52.1%). Only hip fracture surgeries performed within 2 days from admission were analyzed in sensitivity analyses. Using propensity score matching, researchers matched screened patients (mean [SD] age, 84.3 years [7.7 years]; 79.0% female) to 31,941 nonscreened patients (mean [SD] age, 82.1 years [8.7 years]; 78.2% female), resulting in a matched cohort of 25,205 pairs of patients. The estimated overall in-hospital mortality, before propensity matching, was 1.8% (1227 patients). Screened vs nonscreened subjects had no different in-hospital mortality. A total of 25,637 patients were identified from the overall cohort (38.5%) with hip fracture surgeries performed within 2 days of admission, in sensitivity analysis. Findings revealed no association of preoperative echocardiography with reduced in-hospital mortality or postoperative complications.
