Predictors of outcome in the ISCHEMIA-CKD trial: Anatomy vs ischemia

Bainey KR, Fleg JL, Hochman JS, et al. - The extent of coronary artery disease (CAD) predicted death or myocardial infarction (D/MI) among ISCHEMIA-CKD (International Study of Comparative Health Effectiveness with Medical and Invasive Approaches–Chronic Kidney Disease) candidates randomized to the invasive strategy, whereas severity of ischemia did not.
Participants who were randomly assigned to invasive management and had available data on coronary angiography and stress testing were included in the study.
Three hundred seven of the 388 participants had complete coronary angiography and stress testing data.
D/MI occurred in 104/307 candidates (33.9%).
The extent of CAD was linked to an elevated risk of D/MI, although the severity of the ischemia was not. Such associations remained following multivariable adjustment.
The adjusted hazard ratio (HR) for 1VD was 1.86 using 0-vessel disease as reference.
The HR for severe ischemia was 0.84 using moderate ischemia as the reference.
