Predictors of blunt abdominal aortic injury in trauma patients and analysis for mortality
Journal of Vascular Surgery Feb 26, 2019
Sheehan B, et al. - Risk factors for blunt abdominal aortic injury (BAAI) and risk factors for mortality in patients with BAAI were investigated via querying the Trauma Quality Improvement Program (2010-2016) for individuals with blunt trauma. In this largest nationwide series to date, 1,056,633 blunt trauma admissions were identified with BAAI in 1012 (0.1%). A high mortality rate was observed in correlation with BAAI, with hemopneumothorax and inferior vena cava injury associated with the highest risk for mortality. BAAI was encountered in most strong correlation with motor vehicle accident (MVA), followed by pedestrian struck. Rib fracture and trunk abrasion were identified as the other risk factors for BAAI. On encountering these mechanisms of injury, physical examination findings, or image findings, providers are recommended to maintain a high suspicion of injury for BAAI.
