Predictors of adequate lymph node harvest during colectomy for colon cancer

Douaiher J, et al. - Researchers performed this retrospective study using the colectomy-targeted American College of Surgeons National Surgical Quality Improvement Program (NSQIP) for years 2014–2016 to determine the factors affecting the yield of 12 lymph nodes (LN) (a yield recommended by the consensus guidelines) in resections for colon cancer. They analyzed data of 17,612 colectomies for colon cancer, extracted from the NSQIP database. The benchmark of 12 LN harvest was not achieved in around 7.26% colectomies for colon cancer. When resection was done laparoscopically, there was a 74% higher likelihood of achieving 12 LN harvest vs open surgery; it was 72% if hand-assisted vs open surgery. LN harvest was significantly impacted by tumor related factors such as T and N stage, and patient related factors such as gender, age and smoking history.
