Predictors of 10-year stent-related adverse outcomes after coronary drug- eluting stent implantation: The importance of stent size

Kjøller-Hansen L, Kelbæk H, Christiansen EH, et al. - Stent treatment failure (STF) after coronary drug- eluting stent (DES) implantation was predicted by factors such as decreasing stent diameter and increasing stent length together with diabetes, previous revascularization, restenotic, bifurcation, and chronically occluded lesions.
Data were analyzed for 2,073 patients managed with first-generation DES and followed clinically for 10 years.
For a stent diameter ≤2.5 vs ≥3.5 mm, the STF was 23.3 vs 11.8% and target lesion failure (TLF) was 27.9 vs 18.8%.
Stent length <20 vs >40 mm had STF 13.0 vs 29.0% and TLF 18.7 vs 34.6%.
Using hazard ratios for the predictors, a predictive score (PS) was calculated for each lesion.
Lesions with a PS ≤ 1.5 had a 10-year STF rate of 10%; it was 37% in lesions with PS ≥ 3.5.
Large variations in 10-year outcomes existed based on stent size and a few patient and lesion features.
