Predictive value of spot vs 24-hour measures of proteinuria for death, end-stage kidney disease or chronic kidney disease progression

Ying T, et al. - In a prospective study including proteinuric chronic kidney disease (CKD) and kidney transplant recipients, three measures of proteinuria viz, spot urine protein-to-creatinine ratio (UPCR), albumin-to-creatinine ratio (UACR) and 24-h urinary protein excretion (24-UPE), were assessed to determine the measure most predictive of renal events. It was noted that all three measures of proteinuria were similarly predictive of hard clinical endpoints, defined as a composite of death, end-stage kidney disease and > 30% decline in estimated glomerular filtration rate in patients with stable, non-nephrotic CKD. However, less certainty remained as to which measure best predicted the outcomes individually.
