Predictive and prognostic value of optic nerve sheath diameter measurement associated with hyponatremia in Emergency Department
The American Journal of Emergency Medicine Jan 04, 2019
Demir TA, et al. - Researchers examined the association of optic nerve sheath diameter (ONSD) with hyponatremia in symptomatic patients. They analyzed 89 hyponatremia (Na + <135 mmol/L) patients and compared them to 72 patients who have normal serum sodium levels presented to ED at the same time interval. They recorded admission symptoms, serum Na + level, and pre-treatment and post-treatment ONSD in participants with mean age 62.3 ± 17.6 years, and 55.1 ± 20.0 years of the control group. Relative to controls, pre-treatment and post-treatment ONSDs remarkably differed in the patient group. The admission sodium level and ONSD showed a significant negative correlation in the patient group. The area under the curve was 0.870; 0.49 mm was the cut-off level calculated for hyponatremia, which had a sensitivity of 81% and a specificity of 81.9%. In the Emergency Department, changes reflected on ultrasonic imaging of ONSD measurement appeared to be consistent with ICP changes in hyponatremia and change in serum sodium.
