Predicting transient hypocalcemia in patients with unplanned parathyroidectomy after thyroidectomy

Philips R, et al. - In patients with unplanned parathyroidectomy during total or completion thyroidectomy, researchers investigated the efficacy of rapid parathyroid hormone (PTH) values in the prediction of transient postoperative hypocalcemia. Risk factors for unplanned parathyroidectomy included central neck dissection and malignancy. There was also a higher likelihood of hypocalcemia and hypoparathyroidism in those having an unplanned parathyroidectomy. The only significant predictor of hypocalcemia in these individuals was PTH value. They observed that initial calcium supplementation ≥ 1,000 mg lowered the risk of hypocalcemia in candidates with a postoperative PTH of ≤15.
