Predicting stroke in patients without atrial fibrillation

Steensig K, et al. - In patients without atrial fibrillation (AF), researchers ascertained if the CHA2DS2-VASc score can predict AF risk and thromboembolic events. Patients who underwent coronary angiography between 2004 and 2012 were grouped based on CHA2DS2-VASc score. Ineligible patients included those with AF, those who were receiving anticoagulant therapy, and those with follow-up < 30 days. There were 78,233 patients with group sizes varying between 8,299 (CHA2DS2-VASc score > 4) and 19,882 (CHA2DS2-VASc score of 2). Among patients who underwent coronary angiography, CHA2DS2-VASc score predicted a future diagnosis of AF and the composite risk of ischemic stroke, transient ischemic attack, or systemic embolism in patients without AF. In a patient with AF, a CHA2DS2-VASc score of 3 was related to a risk that would justify prophylactic oral anticoagulation treatment.
