Predicting biochemical recurrence after radical prostatectomy: The role of prognostic grade group and index tumor nodule

Vau N, Henriques V, Cheng L, et al. - In this cohort study, 144 consecutive people treated by laparoscopic radical prostatectomy were involved in order to examine whether the grade group evaluated in the index tumor nodule prognosticated biochemical recurrence following surgery. Type of radical prostatectomy (with/without lymphadenectomy), pT and pN status, histologic type of prostate carcinoma (acinar vs mixed histology), surgical margin resection status, perineural invasion, lymphovascular invasion, biochemical recurrence status, presence of tertiary Gleason 5 pattern, and grade group that was evaluated both in overall prostate cancer and in index (dominant) tumor nodule, were assessed. At a mean follow-up time of 12.2 months, 20 persons encountered postoperative biochemical recurrence. Results approved that independent predictor of biochemical recurrence-free survival was grade group in the index tumor nodule, and hence highlighted the significance of reporting grade group in index tumor nodule. The relatively low number of cases in the prevailing series was the main limitation of the study implying the necessity of huge corroborative studies.
