PRECICE intramedullary nail in the treatment of adult leg length discrepancy

Cosic F, et al. - Researchers examined outcomes of 21 adult patients [Mean age: 36.4 years (range 21–65); 19 patients male] who were operated by a single surgeon, using the PRECICE nail, for lower limb length discrepancies. Via a limited approach, low energy femoral osteotomies were done. Using a percutaneous technique and Gigli saw, tibial osteotomies were done. They used a distraction rate of 0.75 mm/day as a standard protocol with a change in the rate of 0.25 mm/day implemented as needed. Among patients, 17 femoral and four tibial lengthenings were done. During mean follow up of 15.1 months (range 6–30), all patients obtained correct lengthening (mean gain 36.5 mm, range 18–80 mm) and all patients consolidated their regenerate bone (mean 268 days, range 99–825). Delayed consolidation was observed in correlation with increasing age, number of prior operations, and smoking. Outcomes suggest that in skeletally mature patients, the PRECICE intramedullary nail is a reliable limb lengthening device, rendering predictable lengthening and bone regeneration.
