Navaneethan SD, et al. – In order to disseminate chronic kidney disease (CKD) stageÂspecific goals of care and education, experts planned to develop a navigator program for patients with CKD and an electronic health recordÂbased enhanced personal health record. A patient navigator program and an enhanced personal health record for the CKD population were successfully developed. No differences were observed in estimated glomerular filtration rate (eGFR) decline and other outcomes among the study groups.
