Postresuscitation care after out-of-hospital cardiac arrest: Clinical update and focus on targeted temperature management

Kirkegaard H, et al. - Researchers discuss the best strategy for managing unconscious out-of-hospital cardiac arrest patients in the hospital setting, with a special emphasis on targeted temperature management. A secured airway is required in all such cases, and hemodynamic support with fluids and/or vasopressors is also a requisite in most of the patients. For unconscious out-of-hospital cardiac arrest patients, as quickly as possible initiation of targeted temperature management is recommended. Maintaining it at 32° to 36°C for at least 24 h, and not increasing rewarming more than 0.5°C per hour are recommended by international guidelines. A principal component of postresuscitation care is delayed and multimodal prognostication. Most of the patients can experience good outcomes with modern postresuscitation care but such care needs major diagnostic and therapeutic resources and specific training. The implementation of regional prehospital resuscitation systems with integrated and specialized cardiac arrest centers is strongly advised by recent international guidelines.
