Postprandial bile acid levels in intestine and plasma reveal altered biliary circulation in chronic pancreatitis patients

Humbert L, et al. - In chronic pancreatitis (CP) patients with exocrine pancreatic insufficiency (EPI), researchers investigated bile acid (BA) secretion and circulation. For this purpose, they used LC-MS/MS and measured postprandial levels of individual BAs in duodenal contents and blood plasma simultaneously from CP patients and healthy volunteers (HV) who were intubated with gastric and duodenal tubes before the administration of a test meal and continuous aspiration of duodenal contents. CP patients vs HV had considerably higher primary to secondary BA ratio as well as increased BA plasma concentration, suggesting an impaired transformation of BAs by gut microbiota and a cholangiohepatic shunt of BA secretion, respectively. In CP patients, the main biliary duct stenosis could lead to alterations of BA circulation and levels, which could aggravate the consequences of EPI on lipid malabsorption.
