Postoperative atrial fibrillation and long-term risk of stroke in patients undergoing liver transplantation

Koshy AN, Enyati A, Weinberg L, et al. - In patients undergoing liver transplantation (LT), researchers conducted this retrospective cohort study to evaluate a possible correlation of postoperative atrial fibrillation (POAF) with subsequent thromboembolic events. Between 2010 and 2018, consecutive adults undergoing LT were analyzed. POAF occurred in 47 (10.2%t) out of the 461 patients included at a median of 3 days after transplantation. Advanced age, postoperative sepsis, and left atrial enlargement were independent predictors of POAF. Twenty-one cases of stroke and systemic embolism occurred over a median follow-up of 4.9 (interquartile range, 2.9–7.2) years. An 8-fold increased risk of thromboembolic events is associated with POAF following LT, and risk stratification of these patients can be done via the CHA2DS2VASc score. Prospective studies are warranted in order to determine whether oral anticoagulants can reduce the risk of thromboembolism after LT.
