Postoperative acute necrotizing pancreatitis of the pancreatic remnant (POANP): A new definition of severe pancreatitis following pancreaticoduodenectomy

Globke B, Timmermann L, Klein F, et al. - In this investigation, researchers assessed parameters of patients undergoing completion pancreatectomy (CP) following initial pancreaticoduodenectomy (PD) and compared those with or without necrosis of the pancreatic remanent. From a prospectively collected database, patients who had CP following PD between January 2005 and December 2017 were identified. Histologically detected postoperative acute necrotizing pancreatitis (POANP) in 33 of 79 patients following CP. Findings suggested that patients who require CP after PD for POANP have an enhanced risk of significant complications and a longer stay in the hospital. The levels of CRP, APACHE II and SOFA appear to correlate with the severity and may predict POANP. Universally accepted definitions with a clinically validated severity grading system for POAP and POANP are required to facilitate appropriate treatment strategies and allow future studies to be compared.
