Posterior or single-stage combined anterior and posterior approach decompression for treating complex cervical spondylotic myelopathy coincident multilevel anterior and posterior compression

Zhou X, et al. - Researchers herein performed a retrospective comparison of surgical treatment methods for cervical spondylotic myelopathy coincident multilevel anterior and posterior compression, known as complex cervical spondylotic myelopathy (cCSM). In terms of clinical outcomes and complications, following were compared: the posterior approach laminoplasty and single-stage anterior approach laminoplasty combined with anterior cervical corpectomy and fusion and anterior cervical discectomy and fusion. Majority of patients with multilevel cCSM experienced significant neurological improvement and pain reduction following posterior approach laminoplasty and single-stage combined approach. Both approaches showed similar results in terms of decompression and neurological improvement. Notably, as regards surgical costs, surgical time, blood loss, and complication rate, the superiority of the posterior approach over the combined approach was obvious.
