Positive oral contrast solution at MDCT for suspected acute appendicitis in adults: Rate of appendiceal luminal filling of normal and inflamed appendixes

Tang SJ, et al. - Via a retrospective review of MDCT in 684 consecutive adult persosns with suspected acute appendicitis in a 19-month period, experts evaluated the rate of appendiceal filling with a positive oral contrast solution at MDCT done for suspected acute appendicitis in adults. In 313/519 individuals, cecal opacification was validated, with no variation between those acknowledged to be positive or negative for appendicitis. In none of the 68 with appendicitis and in 205 of the 245 without appendicitis, when positive oral contrast solution reached the cecum, appendiceal filling was noticed. Amongst CT colonography (CTC) controls, appendiceal filling was comparable to the cohort recognized to be without appendicitis (2,240/2,552). Therefore, in MDCT for suspected acute appendicitis, when positive oral contrast solution reaches the cecum, luminal filling of the noninflamed appendix exceeds 80% implying results comparable to screening CTC. The appendix did not fill in proven acute appendicitis, suggesting appendiceal filling could permit exclusion of appendicitis with great certainty. These results imply in cases of suspected acute appendicitis positive oral contrast solution may raise diagnostic exactitude and confidence.
