Positive margins after mastectomy in patients with invasive lobular carcinoma of the breast: Incidence and management strategies

Hewitt KC, Miller P, Piper M, et al. - Researchers aimed at determining the positive margin rate after mastectomy for invasive lobular carcinoma. The positive margin rate was retrospectively determined in 357 patients with stage I-III invasive lobular carcinoma (ILC) undergoing mastectomy. Overall positive margin rate of 10.6% was recorded; those with T3 tumors had a positive margin rate of 18.7%. They recorded significantly improved outcomes in correlation with receiving additional local therapy for positive margins when compared with receiving systemic therapy alone. Findings overall suggest that even following mastectomy, a high proportion of patients with large ILC tumors do not exhibit clear margins. This warrants testing neoadjuvant strategies even prior to planned mastectomy.
