Poor compliance with postvasectomy semen testing: Analysis of factors and barriers

Bradshaw A, et al. - Given postvasectomy semen analysis (PVSA) was advised by American Urological Association Guidelines to ensure procedure success, but poor compliance is suggested by current literature, so, researchers measured PVSA compliance and evaluated obstacles to completion. They conducted a retrospective review of vasectomies at San Diego Veterans Administration Hospital and UC San Diego Health between 2006 and 2018. Patients were provided with preprocedural counseling concerning semen analysis necessity. Follow-up visit within 2-4 weeks and semen analysis following 15-20 ejaculations were incorporated in the postprocedural management. The only factor related to noncompliance of semen analysis was fatherhood, as revealed in the multivariable analysis. Distance (38%), time constraints (34%), and forgetfulness (23%) were reported as the main obstacles to semen analysis completion among men interviewed. Increased probability of completion with home-based semen testing was stated by 92%. Findings revealed poor PVSA compliance among patients despite preprocedural counseling. Improved quality of care for men undergoing vasectomy may be ensured by including home-based semen testing.
