Poor accordance to a DASH dietary pattern is associated with higher risk of ESRD among adults with moderate chronic kidney disease and hypertension

Banerjee T, et al. - Among enrollees of the National Health and Nutrition Examination Survey (1988-1994), researchers investigated the association of a Dietary Approaches to Stop Hypertension (DASH) diet with risk of end-stage renal disease (ESRD). Participants were 1,110 adults aged ≥ 20 years with hypertension and chronic kidney disease (CKD) (estimated glomerular filtration rate, eGFR 30-59 ml/min/1.73 m2). They used a 24-hour dietary recall questionnaire to determine baseline DASH diet accordance score. Through linkage to the U.S. Renal Data System registry, they determined ESRD. After adjusting for sociodemographics, clinical and nutritional factors, eGFR, and albuminuria, the relative hazard (RH) for ESRD were calculated using the Fine-Gray competing risks method. The development of ESRD was reported in 18.4% of subjects during a median follow-up of 7.8 years. A greater risk of ESRD was observed in association with a low accordance to a DASH diet in adults with moderate CKD and hypertension; this was especially seen in non-Hispanic blacks and diabetics.
