Pneumococcal serotype trends, surveillance and risk factors in UK adult pneumonia, 2013–18

Pick H, Daniel P, Rodrigo C, et al. - Researchers analyzed adults who were admitted for community-acquired pneumonia (CAP) in two large university hospitals from September 2013 and August 2018, to determine the shifts in the serotypes involved in adult pneumococcal pneumonia and the patient groups related to vaccine-type disease during this time period of last 5 years, via this population-based prospective cohort analysis. They used a new 24-valent urinary monoclonal antibody assay and blood cultures to detect pneumococcal serotypes. They compared trends in incidence rates against national invasive pneumococcal disease (IPD) data. The presence of pneumococcal pneumonia was detected in 1,075 (36.6%) of 2,934 adults hospitalised with CAP. Findings revealed a rising incidence of pneumococcal CAP, mainly attributable to non-vaccine type serotypes and serotype 3. A broad similarity was noted between observed incidence trends and IPD data. They also found that 349 (32.5%) and 431 (40.1%) patients with pneumococcal pneumonia had PCV13non7 (56.9% serotype 3) and PPV23non13 (44.1% serotype 8) serotypes, respectively. The likelihood of PPV23-serotype pneumonia was more in adults outside currently recognized clinical risk groups.
