Planned mode of delivery after previous cesarean section and short-term maternal and perinatal outcomes: A population-based record linkage cohort study in Scotland

Fitzpatrick KE, et al. - Researchers examined the short-term maternal and perinatal outcomes correlated with a planned mode of delivery after previous cesarean section among women delivering a term singleton and considered eligible to have a planned vaginal birth after previous cesarean. Using linked Scottish national datasets, they conducted a population-based cohort study of 74,043 term singleton births in Scotland between 2002 and 2015 to women with one or more previous cesarean sections. As per findings, the risk of having serious birth-related complications such as uterine rupture and blood transfusion, as well as the risk of adverse perinatal outcomes increases in correlation to attempting a vaginal birth vs having an elective repeat cesarean section. Conversely, an increased likelihood of breastfeeding was observed in correlation with attempting a vaginal birth, whereas there appeared differences in the association with other maternal outcomes according to whether a woman has any previous vaginal deliveries and the number of previous cesarean sections she has had. However, for either delivery approach, small absolute risk of adverse maternal and perinatal outcomes was observed: overall, serious maternal complications were evident in just 1.8% of those attempting a vaginal birth and 0.8% of those having an elective repeat cesarean section, and one or more of the adverse perinatal outcomes considered were observed in 8.0% of those attempting a vaginal birth and 6.4% of those having an elective repeat cesarean section.
