Persistent, refractory, and biphasic anaphylaxis: A multidisciplinary Delphi study

Dribin TE, Sampson HA, Camargo CA, et al. - Researchers utilized a multidisciplinary group of clinical and research experts in anaphylaxis in order to develop consensus definitions for clinically relevant anaphylaxis results. They created agenda topics as well as drafted queries to review during monthly conference calls, employing Delphi methodology. Via online surveys, a 19-member panel was asked to rate their level of agreement with the appropriateness of statements on a scale of 1 to 9. Then, a disagreement index was computed with values less than 1.0 grouped as “consensus reached.” Not only for persistent, refractory, and biphasic anaphylaxis but also for persistent and biphasic nonanaphylactic reactions, the panel developed outcome definitions. Panel members showed consensus with respect to the requirement to construct an anaphylaxis severity grading system. In clinical care and research, dissemination as well as use of these definitions will help standardize the terminology applied to define anaphylaxis results and serve as the foundation for future inquiry, including research focused on developing an anaphylaxis severity grading system.
