Perioperative systemic dexamethasone reduces length of stay in total joint arthroplasty: A systematic review and meta-analysis of randomized controlled trials

Lex JR, Edwards TC, Packer T, et al. - This study was undertaken to investigate the impact of perioperative systemic corticosteroids at varying doses and timings on early postoperative recovery outcomes following unilateral total knee (TKA) and total hip arthroplasty (THA). The primary outcome was the length of stay (LOS). A systematic review and meta-analysis of randomized controlled trials were conducted to search MEDLINE, Embase, and Cochrane Library databases from inception to June 1, 2020. Researchers included a total of 17 studies including a total of 1,957 patients. Peri-operative corticosteroids were found to be safe, facilitate earlier discharge and improve patient recovery following unilateral TKA and THA. It was reported that higher doses (15-20mg of dexamethasone) are correlated with further reductions in dynamic pain and PONV, and repeat dosing may further decrease LOS.
