Perioperative SBP changes during orthopedic surgery in the elderly: Clinical implications

Koutsaki M, et al. - In this investigation, researchers identified changes in SBP during the perioperative period of a scheduled knee surgery under regional anesthesia and the extent of perioperative (in-hospital) white-coat effect. During both cardiological and anesthesiological visits, all patients, aged ≥60 years, underwent clinic SBP measurements, while home SBP was acquired the week before admission. The significant white-coat effect found in scheduled noncardiac surgery is clinically essential and the patient's hypertensive burden is strongly reflected in the home BP measurement conducted before surgery or ABPM. Particularly in patients aged more than 75 years, blood pressure decrease during surgery is quite pronounced. It is important to avoid aggressive BP lowering.
