Perioperative anesthetic and transfusion management of veno-venous extracorporeal membrane oxygenation patients undergoing noncardiac surgery: A case series of 21 procedures

Fierro MA, et al. - In this retrospective, nonrandomized, observational study performed in a single, university-affiliated, quaternary medical center, researchers assessed the perioperative management of veno-venous extracorporeal membrane oxygenation (VV ECMO), focusing on perioperative challenges associated with hemodynamic instability, impaired gas exchange, bleeding, and coagulopathy, in 14 patients who had 21 noncardiac surgical procedures during the period of January 1, 2014, through April 1, 2016. According to the findings, the use of either inhalational or intravenous anesthetics was valid to anesthetize VV ECMO patients. Frequent interventions were needed for patient hemodynamics, oxygenation, and decarboxylation, but these could typically be optimized to meet clinically acceptable thresholds.
