Performance of basophil activation test and specific IgG4 as diagnostic tools in non-specific lipid transfer protein allergy: Antwerp-Barcelona-comparison

Decuyper II, Pascal M, Van Gasse AL, et al. - Researchers compared both clinical as well as in vitro nsLTP allergy features and diagnostic performance of a basophil activation test (BAT) and sIgG4 in nsLTP-sensitized patients from Antwerp (ANT, Belgium) and Barcelona (BCN, Spain). They found that clinically important Mal d 3-sensitization was identified by only ratios of sIgG4/sIgE rMal d 3 and BAT rMal d 3 (0,001µg/mL) in BCN, with a sensitivity and specificity of 60-63% and 75-67% respectively. With a sensitivity 60% and specificity 83%, the added value was displayed by only the sIgE/total IgE rPru p 3 ratio in ANT. More sensitivity to lower allergen levels was noted in symptomatic patients in BCN vs ANT. More commonly, ANT-patients got sensitized to pollen. Across regions, NsLTP-related allergy profiles and diagnostic performance varied significantly and was component-specific. The possibility of basophil sensitivity showing geographical variations was also suggested.
