Percutaneous CT guided bone biopsy for suspected osteomyelitis: Diagnostic yield and impact on patient’s treatment change and recovery

Hoang D, et al. - Researchers evaluated 115 patients (mean age 50.86 ± 14.49 years) who had a percutaneous CT guided bone biopsy (PCBB) for suspected osteomyelitis (OM) in order to determine its value for diagnosis in these patients. Further, they determined its eventual effect on patient management and recovery. A positive blood culture was noted in 22/91 (24%) and a positive wound culture was reported in all patients (23/23). In 84.1% and 14.2%, definitive and possible OM were reported, respectively, on imaging; 1.8% was reported as having no OM. There were 24/115 (21%) that had a positive bone biopsy culture and 10/24 (42%) had total positive bone cultures influenced the treatment plan. Diagnostic yield of culture was not significantly influenced by antibiotics. They identified no statistical significance when comparing treatment change based on bone culture results vs all other factors combined, or when assessing clinical improvement with vs without positive bone cultures. Bone biopsy yield of positive culture is low, and it leads to a minor influence for varying the treatment plan or changing the course of patient recovery, despite positive cross-sectional imaging findings of OM.
