Pediatric and adult ambulatory blood pressure thresholds and blood pressure load as predictors of left ventricular hypertrophy in adolescents

Hamdani G, Mitsnefes MM, Flynn JT, et al. - In order to investigate the correlation of different ambulatory blood pressure (BP) parameters with hypertensive target organ injury, researchers sought to compare adult and pediatric cut points and evaluate the utility of BP load as a predictor of left ventricular hypertrophy (LVH) in 327 adolescents who were ≥ 13 years of age. They used logistic regression to evaluate the correlation of different ambulatory BP parameters with LVH. Data reported that 68 (20.8%) candidates had LVH. The authors conclude that a single static cut point using an absolute ambulatory systolic BP value is similar to sex-and height-based systolic BP percentiles in anticipating LVH and that BP load provides no additional (to mean systolic BP) value in predicting LVH in adolescents.
