Patterns of utilization and outcome of ovarian conservation for young women with minimal-risk endometrial cancer
Gynecologic Oncology May 02, 2019
Mandelbaum RS, et al. - In this population-based retrospective analysis, researchers used the Nationwide Inpatient Sample (2007-2015) to profile patient features related to and consequences of ovarian conservation at the time of hysterectomy in young women (aged <50) with minimal-risk endometrial cancer. They compared 2314 participants who had ovarian conservation to 8191 who had oophorectomy. They identified nine distinct patterns of patient features. The range for ovarian conservation rates was from 11.7% (women aged 40–49 who had abdominal hysterectomy at an urban teaching hospital) to 60.5% (non-obese women aged <40 with median household income ≥$63,000). Following propensity score matching, a significant association of ovarian conservation with a decreased probability of hospitalization >2 days was observed. The two groups did not differ in terms of rates of surgical complications. Findings revealed an association of ovarian conservation with decreased length of hospitalization in the benign gynecologic disease and cervical cancer cohorts.
