Pathologic findings in fallopian tubes of women with chronic pelvic pain after Essure placement
Contraception Apr 30, 2020
Rubin A, Chen JF, Veade A, et al. - Researchers sought to describe histopathologic findings in long-term Essure users with chronic pelvic pain via performing descriptive evaluation and comparison of histopathologic features of hysterectomy specimens removed for a primary diagnosis of chronic pelvic pain from women with (n = 3) and without (n = 3) prior placement of Essure coils (mean of 8.6 years prior). Fibrosis was evident in interstitial fallopian tubes of Essure patients. Two cases had flattening of ampullary epithelial folds. No acute inflammation was observed in tubes of Essure users, however, focal chronic inflammation was noted in 2/3. Additional findings, such as endometriosis, adenomyosis or leiomyomas, were reported in all patients that could be linked with pain. Symptoms may more plausibly be ascribed to confounding gynecologic conditions or other mechanisms in view of the minimal and bland inflammation in Essure cases.
