Pathologic findings and clinical outcomes in women undergoing risk-reducing surgery to prevent ovarian and fallopian tube carcinoma: a large prospective single institution experience

Rush SK, Swisher EM, Garcia RL, et al. - Given women at increased risk of ovarian, fallopian tube (FT), and peritoneal carcinoma (collectively OC) are advised to undergo risk-reducing salpingo-oophorectomy (RRSO), researchers undertook this inquiry in the largest single-institution prospective cohort comprising women undergoing RRSO, including those having mutations in non-BRCA homologous repair genes, to determine the rates of occult neoplasia in these women. They found the highest risk of occult neoplasia at RRSO in BRCA1 carriers, and the frequency raised with age. Commonly, women with BRCA1/2 mutations undergo RRSO beyond advised ages. A new finding was the detection of FT intraepithelial neoplasia in one PALB2 carrier. To detect occult neoplasia, serial sectioning is important and is recommended to be performed for all risk-reducing surgeries.
