Parathyroidectomy for patients with primary hyperparathyroidism and associations with hypertension
JAMA Jan 24, 2020
Graff-Baker AN, et al. - In this cohort study and retrospective database review, investigators explored whether parathyroidectomy improves hypertension in individuals with primary hyperparathyroidism (PHPT). They distinguished individuals with PHPT and hypertension between January 1, 2008, and December 31, 2016. They compared the mean arterial pressure (MAP) and number of antihypertensive medications between those who did and did not undergo parathyroidectomy. They applied multivariable logistic regression to evaluate the adjusted odds ratios for both increased and decreased use of antihypertensive medications. Parathyroidectomy may be correlated not only with greater decreases in their MAP but also with reduced requirements for antihypertensive medications among hypertensive patients with PHPT. Results showed that parathyroidectomy reduced the number of individuals who began taking antihypertensive medications. Further study will be needed to evaulate whether there are downstream cardiovascular advantages of parathyroidectomy.
