Paraproteinemic keratopathy associated with monoclonal gammopathy of undetermined significance (MGUS): Clinical findings in twelve patients including recurrence after keratoplasty

Skalicka P, Dudakova L, Palos M, et al. - Ocular findings of 12 individuals with paraproteinemic keratopathy associated with monoclonal gammopathy of undetermined significance (MGUS) were reported. Corneal spectral domain optical coherence tomography was performed for ocular examination. Using conventional Sanger sequencing, researchers screened the TGFBI gene in three individuals with an initial diagnosis of a lattice or Thiel–Behnke corneal dystrophy. MGUS was confirmed by systemic examination and serum protein electrophoresis in 12 individuals (9 males and 3 females); the mean age at presentation was 52.2 years (range 24–63 years). The best-corrected visual acuity at presentation ranged from 1.25 to 0.32. All individuals exhibited bilateral corneal opacities hence they suggest considering a diagnosis of MGUS in patients with bilateral corneal opacities. The appearance can mimic corneal dystrophies or cystinosis. As per observations, the systemic treatment of MGUS seems incapable of preventing recurrence of paraproteinemic keratopathy following keratoplasty.
