Packed red blood cell donor age affects overall survival in transfused patients undergoing hepatectomy for non-hepatocellular malignancy

Loftus TJ, et al. - As packed red blood cell (PRBC) transfusion has been associated with worse oncologic outcomes among patients undergoing hepatectomy, researchers investigated the influence of PRBC donor age on survival after hepatectomy for non-hepatocellular malignancies. They retrospectively evaluated 149 patients [76 received a perioperative PRBC transfusion (median 2 units)] who underwent hepatectomy for non-hepatocellular malignancy from 2005-2014. Transfusion was noted to be correlated with increased median length of stay and median operative blood loss vs non-transfused, respectively. In transfused patients, decreased recurrence-free survival and overall survival was observed in correlation with receipt of PRBC from older donors compared to younger. Multivariate analysis yielded PRBC donor age as an independent predictor of decreased recurrence free survival.
