Overresuscitation with plasma is associated with sustained fibrinolysis shutdown and death in pediatric traumatic brain injury

Leeper CM, et al. - Researchers investigated the indication for transfusion, fibrinolysis status, and outcomes after plasma transfusion in injured pediatric patients. Findings revealed an independent association of plasma transfusion with sustained fibrinolysis shutdown (SD). In addition, severe traumatic brain injury (TBI) is noted to have a correlation with sustained SD. An extremely poor prognosis was observed in relation to the combined effect of plasma transfusion and severe TBI. They recommend not targetting plasma transfusion to INR thresholds but rather to rapid thromboelastography activated clotting time and clinical bleeding.
