Outcomes of transcatheter vs surgical aortic valve replacement in low-risk patients

Koshy AN, et al. - Researchers sought to establish the safety and efficacy of transcatheter aortic valve replacement (TAVR) in patients with severe aortic stenosis (AS) who were at low surgical risk. They identified four randomized controlled trials (RCTs) in cases at low surgical risk comparing TAVR to surgical aortic valve replacement (SAVR) via performing a systematic review of PubMed, Medline and EMBASE. The four RCTs involved 2,836 patients; 1,363 patients randomized to SAVR and 1,473 to TAVR. Significantly lower rate of the composite of death and stroke was observed with TAVR vs SAVR in patients with severe AS at low surgical risk. They recommend undertaking longer-term follow-up with a focus on the influence of permanent pacemaker (PPM) implantation, paravalvular leak and structural valve deterioration before generalization of the use of TAVR to the broader population of patients with AS.
