Outcomes of sub-threshold abdominal aortic aneurysms undergoing surveillance in patients aged 85 years or over

Elbasty A, et al. - Researchers examined the safety of discontinuation of surveillance in patients aged ≥85 years undergoing abdominal aortic aneurysms (AAAs) surveillance via determining their outcomes. They conducted a retrospective cohort study of 101 patients (88 male; median follow up period 56.0 months; aged 85 years) undergoing surveillance with a small (30–54 mm) AAA between January 2007 and November 2017. Using a threshold of 55 mm, the decision regarding intervention was taken in all patients. Threshold was not reached in the majority of patients (72.3%) undergoing surveillance at the end of follow up. Data suggest the possible safety of discontinuation of aneurysm surveillance in patients aged 85 years with aneurysms < 40 mm. They suggest early assessment of fitness for surgery in patients with a larger aneurysm or those approaching threshold to prevent unnecessary surveillance. Although complex, the decision to treat aneurysms reaching threshold is appropriate in selected patients.
