Outcomes of primary ileocolic resection for pediatric Crohn disease in the biologic era

Spencer EA, Jarchin L, Rolfes P, et al. - In this analysis of a pediatric Crohn disease (CD) cohort who had ileocolic resection (ICR), it was realized that postoperative surveillance and effective prophylaxis would be beneficial in view of the inherent risk of postoperative recurrence linked with age and disease duration.
In this single-center, retrospective study, 78 pediatric (<18 years) CD patients who underwent ICR a median of 17.8 months (interquartile range [IQR] 2.6–53.9) from diagnosis were analyzed.
More than 1 post-operative endoscopy was performed in 41 patients, and in these cases, the endoscopic recurrence rate at 2 years was 46% (median time to recurrence: 10 [7–20] months).
Histologic recurrence was evident in 44% in endoscopic remission.
Factors associated with endoscopic recurrence were: younger age at diagnosis and longer disease duration.
Rate of 30-day complications was 18%; only 1% suffered severe complications.
Post-surgery, significant improvement in all anthropometric measures was observed.
