Outcomes in young adults with tetralogy of Fallot and pulmonary annular preserving or transannular patch repairs

Ducas RA, Harris L, Labos C, et al. - In early surgical tetralogy of Fallot (ToF) repair, patching across the pulmonic annulus (TAP) was involved, which resulted in severe pulmonic regurgitation. It is assumed that modifications preserving the pulmonic annulus (AP) may improve long-term outcome. Researchers here examined the necessity for late reintervention in adults with AP repair and those with TAP repair. They performed a retrospective review of 230 adults (born 1981-1996) with childhood intra-cardiac ToF repairs; 104 with AP repair and 126 with TAP repair. Observations revealed significantly fewer late re-interventions among patients who had annular preserving ToF repair vs TAP repair, the majority of reinterventions were due to PVR.
