Outcomes in patients undergoing cardiac re-transplantation: A propensity matched cohort analysis of the UNOS registry

Miller RJH, et al. - Given an increased risk of mortality in correlation to cardiac re-transplantation that accounts for approximately 3% of cardiac transplantation, researchers examined if re-transplantation was associated with all-cause mortality after adjusting for factors inherent to re-transplantation including previous sternotomy, sensitization and renal dysfunction. From the United Network for Organ Sharing database, patients undergoing cardiac re-transplantation were identified based on transplant listing diagnosis and history of the previous transplant. Among 62112 heart transplant recipients (mean age 46.6 ± 19.1 years), 2202 (3.4%) underwent late cardiac retransplantation (>1 year after initial transplant and not for acute rejection). After matching for comorbidities, no increase in all-cause mortality was observed in correlation to late re-transplantation in the adult population. Findings thereby emphasize assessing indication acuity and co-morbid conditions as significant when considering re-transplant candidacy.
