Outcomes following extra-articular fractures of the scapula: A systematic review

Bi AS, et al. - Researchers conducted the largest systematic review compiling the existing literature on operative and conservative treatment of extraarticular fractures of the scapula. Searching multiple electronic databases according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) statement, they identified 42 manuscripts with a total of 669 patients (average age: 41.2 years; 78.1% male) for inclusion in this analysis. A blunt force mechanism was reported in 98.8%, and other injures outside the scapula were reported in 66.7% at the time of presentation. Excellent outcomes were achieved in patients with extraarticular fractures of the scapula following either nonoperative or operative management. Operative treatment of extra-articular scapular fractures resulted in union rates ranging from 94.7% – 100%, complication rates of 14.4% and Constant scores of 84.4. Conservative treatment of extra-articular scapular fractures led to union rates from 75% – 90.2%, complication rates of 3.4% and Constant score of 79.0. Forward elevation greater than 150° was identified in all extra-articular scapular fractures at final follow up regardless of operative vs non-operative treatment.
