Outcome of pregnancy after renal transplantation

Mohamed Hassan S, et al. - This study was designed to compare the outcome of pregnancy in patients who became pregnant within 24 months of renal transplantation and patients who became pregnant more than 24 months after renal transplantation. Findings revealed better pregnancy outcome in association with a longer interval between renal transplantation and pregnancy.
Methods
- Forty-four patients who became pregnant following renal transplantation comprised the study cohort.
- Living donors were used for renal transplantation in all cases.
- In group A, 24 patients who became pregnant more than 24 months after renal transplantation were allocated and in group B, 20 patients who inadvertently became pregnant within 24 months of renal transplantation were allocated.
- Researchers measured serum creatinine and 24-hour urinary protein concentration each trimester.
- They determined the incidences of preeclampsia and gestational diabetes, the timing and mode of delivery, the rate of preterm labor, and the mean fetal birth weight.
Results
- In groups A and B, the mean gestational ages were 35.8±3 weeks and 34.1±2.5 weeks, respectively.
- The mean fetal birth weights were 2,480±316 g and 2,284.5±262 g, respectively, for the groups.
- These differences were observed to be statistically significant.
- In groups A and B, the incidences of preterm labor were 45.8% and 55%, respectively.
- Group B showed significantly higher proteinuria during the third trimester of pregnancy.
- In groups A and B, preeclampsia occurred in 25% of the cases and 30% of the cases, respectively; this difference was not statistically significant.
- In 2 out of 24 cases in group A and 2 out of 20 cases in group B, gestational diabetes occurred.
- Normal vaginal delivery occurred in 58.3% of cases in group A and 55% of cases in group B, and cesarean section was performed in 41.6% and 45% of cases, respectively.
