Outcome of pregnancies in women with pulmonary hypertension: A single-centre experience from South India

Keepanasseril A, et al. - Researchers performed a retrospective observational study of women with pulmonary hypertension, attending a tertiary center in south India, for assessing maternal complications and pregnancy outcome among these women. They identified 73 women with pulmonary hypertension reporting 81 pregnancies. Pulmonary hypertension secondary to congenital heart disease was reported in the majority of these women (80.8%); Eisenmenger syndrome was reported in 17.8%. In 25 (31.3%) pregnancies, administration of advanced pulmonary artery hypertension (PAH) medication, sildenafil, was reported. Four maternal deaths were recorded; of these, three had Eisenmenger syndrome. Heart failure complicated 6.3% and fetal growth restriction 26.3% of pregnancies. These findings highlight the concerns regarding mortality in pregnant women with pulmonary hypertension despite advances in care, especially in those with Eisenmenger syndrome. They noted that even in low- to middle-income countries with limited resources, multidisciplinary team management in tertiary centers and the use of advanced PAH medications could result in a reduction in morbidity and mortality related to pulmonary hypertension.
