Optimizing humeral stem fixation in revision reverse shoulder arthroplasty with the cement-within-cement technique

Gorman RA, Christmas KN, Simon P, et al. - This study was sought to present on the clinical outcomes of patients undergoing revision reverse shoulder arthroplasty (RSA) by the cement-within-cement technique, as well as to distinguish if surgical technique can influence subsequent humeral loosening. Researchers enrolled a total of 98 patients, cemented humeral components that were revised to RSA applying the cement-within-cement technique. Eight patients were compared in whom humeral stem loosening developed with 90 patients whose stem remained fixed. The findings showed that in revision RSA, usage of the cement-within-cement technique for fixation of the humeral component was found to be effective in improving functional outcome scores and shoulder range of motion. Moreover, these data imply that efforts to maximize the cement volume during reimplantation may lessen the chance of humeral stem loosening requiring additional revision.
