Optimizing atherosclerotic cardiovascular disease risk estimation for veterans with diabetes mellitus
Circulation: Cardiovascular Quality and Outcomes Sep 27, 2020
Raghavan S, Ho YL, Vassy JL, et al. - Provided that the predicted 10-year risk of atherosclerotic cardiovascular disease (ASCVD) in patients with diabetes mellitus is used to direct primary prevention, however, the performance of risk estimators (2013 Pooled Cohort Equations and Risk Equations for Complications of Diabetes [RECODe]) varies across populations, researchers assessed risk equations for initial ASCVD events in US veterans with diabetes mellitus and improved model performance in this population. The sample consisted of 183,096 adults with diabetes mellitus and without prior ASCVD who received care in the Veterans Affairs Healthcare System (VA) from 2002 to 2016 with a mean follow-up of 4.6 years. In veterans with diabetes mellitus, current ASCVD risk equations overestimate the risk, potentially affecting guideline-indicated statin therapy. Using readily accessible electronic health record data, prediction model efficiency can be enhanced for a health system's patients.
