Optimal cut-off age in the TNM Staging system of differentiated thyroid cancer: Is 55 years better than 45 years?

Kim M, et al. - In 6,333 patients with differentiated thyroid cancer (DTC) who underwent thyroid surgery at two tertiary referral centers between 1996 and 2005, researchers assessed the optimal cut-off age for predicting disease-specific survival (DSS). The optimal cut-off age for the prediction of DSS was evaluated between 45 and 65 years. Investigators found that the median age of patients was 46.0 years and 5,498 (87%) were female. It was noted that the median follow-up period was 10.0 years, and 10-year DSS rate was 98%. According to this cohort study, the cut-off age of 55 years was better suited for tumor, node, metastases (TNM) staging to achieve better DSS predictability in patients with DTC. This change would prevent over-staging and over-aggressive treatment in low-risk patients.
