Acuna SA, et al. - Researchers performed a comparison between operative strategies (the Hartmann procedure, primary resection and anastomosis, and laparoscopic lavage) for perforated diverticulitis. They analyzed randomized clinical trials evaluating operative strategies for perforated diverticulitis and determined pooled risk ratios for major complications, reoperation, and mortality using random-effects models. For Hinchey III diverticulitis, laparoscopic lavage is associated with heightened risk of major complications when compared to primary resection. The Hartmann procedure leads to a lower rate of stoma reversal and a higher rate of complications suggesting primary resection and anastomosis be the optimal management strategy for perforated diverticulitis.
