Open rhinoplasty using a columellar strut: Effects of the graft on nasal tip projection and rotation

Bucher S, et al. - Researchers examined the change in nasolabial angles and nasal tip projections following exclusive columellar strut graft implantations and in addition to other surgical steps. Furthermore, they investigated the differences in these parameters between patients with conchal and septal columellar strut grafts. In this monocentric retrospective cohort study of 173 open septorhinoplasties with columellar strut graft technique, performed between 2006 and 2016, they determined the pre- and postoperative nasal tip projection, nasolabial and nasofrontal angle and interalar distance and calculated the Goode-ratio. They observed improvement only in nasal tip rotations and projections when a columellar strut graft was implanted without any additional surgical step; this was specifically observed when the surgeon intended this. In other words, no selection of the strut graft for an uprotational intention did not relevantly influence the nasolabial angle or nasal tip projection. However, the columellar strut graft yet represents an important tool in cases where an uprotation is sought. Given the better results observed with using conchal instead of septal columellar strut grafts, they advise utilizing them for these cases.
